The Jubilee Street Practice is a Safe Surgery, this means that we are committed to providing equal access to our services for everyone in our practice area, regardless of their immigration status. This is in line with our duties under NHS England guidelines for GP registration and informed by our knowledge of the barriers to healthcare faced by migrants in vulnerable circumstances.

We are registering new patients who reside within our catchment area, please check location map to see if you are within our catchment area. Preference is given to patients who do not have a local GP already. Please fill out this online registration form, ensuring that you read carefully and answer all questions. You will be asked your personal details, past medical history and other information to help us trace your existing records. Please provide as much information as possible, as missing information may lead to your registration being delayed or rejected.
If you cannot fill out an online registration form, then please ask at reception for a printed paper copy of the form.
