We are happy that you have chosen The Jubilee Street Practice to look after your healthcare needs, and we look forward to supporting your wellbeing. We are a paperless practice. Booking appointments, asking for prescriptions or looking at your test results are things we ask our patients to do online.
Download the NHS App
If you haven't done so already, please download the NHS App to access our services, whether you're at home or from work. You can use this to order a prescription or look at your test results, without the need to call or visit us in person. If you need help, consider asking a friend or family member to assist you.
We are a patient triage practice
The Surgery is operating a total triage appointment system, using an online consultation tool called eConsult. This tool is available Monday to Friday 07:30-12:00 and 14:00-17:00.
Make sure the details you have given to us are up to date
We are a green practice. It is important that we hold up-to-date contact details for you and your family. Please ensure your details are up to date, and make any changes using the NHS App.
If there are any repeat medication needs, let us know
Sometimes it can take longer than expected for your patient record to be transferred to us from your old care provider. You can let us know here if there are any repeat medications.
If we are your first GP practice
Please provide us with a list of your repeat medications. If you are a parent/legal guardian for a child that is registered to with us, please share their immunisation record.
Let us know if you need an interpreter
We can make sure that an advocate is booked for future appointments.
Keeping in touch
To help us to know how to keep in touch with you and share important updates, please let us know your contact preferences here.
Patient Participation
We hope you will join our patient participation group, which meets regularly to help us better meet our patients' needs.
JSP Community Wellbeing Hub
We also do a lot of work with our local community, and hope you might want to join in, whether by getting your hands dirty in our garden, or attending an exercise class.
We aim to reply to all online consultation request on the same day
We understand that not everybody has the time to wait in a telephone queue or visit the practice. To help with this and to free up our telephone lines for vulnerable people and urgent needs, we offer patient triage online. This allows us to get you the healthcare that you need more quickly.
How to get the medicine you need
We accept prescription requests online only. This is to ensure both safest and quickest access to your medicines.
All repeat prescriptions are requested via your NHS APP.
Signposting
As a GP practice, we know which services are best suited to particular healthcare needs. We can help you to get there more quickly by referring you onto relevant services. All our staff are trained to know when to refer to these services. This includes Community Dental services, Minor Eye Condition services, Community Pharmacy and others.
How to get help when the practice is closed
If you are unwell and feel that your query cannot wait until we re-open, you can contact the Tower Hamlets Out Of Hours GP service on 020 7377 7151. The Tower Hamlets UTC (Urgent Treatment Centre) is open 24 hours, 7 days a week and 365 days.
If you need medical care that's urgent but not life threatening, you can call NHS 111.
For emergency care
In an emergency, attend the Accident and Emergency department at the Royal London Hospital or dial 999 and ask for an ambulance.
If you have chest pain and think you are having a heart attack, or think you (or a relative) may have had a stroke (use FAST), phone an ambulance without delay. Early treatment is important.
Please do not attend A&E unless it is real accident or an emergency!
For urgent mental health problems, contact the psychiatric liaison service at the Royal London Hospital on 020 7377 7000.
For emergency dental service, contact 020 7377 7151.
